Commuter Registration Form - Needed to get started in the program. You will receive a quarterly newsletter from Rideshare. The form can also register a person for carpool matching and Guaranteed Ride Home services.

Incentive Monthly Tracking Form - Once registered in the "Lucky Buck" program, each employee will need to submit a Monthly Tracking Form to collect their incentive points.

Employee Interest Survey - Can be used as a first step to determine which transportation options are most desired by employees at your worksite. It is in MSWord format so it can be changed to suit your needs. Either have employees return the form to you for processing, or return the forms to SLO Rideshare. An Employee Transportation Survey can be used as a follow-up for a more comprehensive analysis of performance and employee interests. Contact us to set up an Employee Transportation Survey.

Spreadsheet for Identifying Quarterly and Annual Awards for Employee Participation in the Transportation Incentive Program.
